In 2015, around 3 of nearly 80 published studies mentioned issues of concern uncovered by FDA

2013, about 2 percent of the nearly 650 inspections the FDA carried out at clinical trial sites were classified as OAI (Offical Action Indicated)

Blockchain enabled immutable records could potentially address issues of tampering and/or selective reporting
